Prior to booking and paying for a consultation, the Client confirms that they have read and understand the following policies:

Bookings
All bookings shall be paid for at the time of booking.
Bookings shall be refundable if they are cancelled at least five (5) days prior to the appointment date.
If a booking is cancelled within five (5) days of an appointment date, the client shall receive no refund.

Client Obligations
You agree to cooperate in completing initial health forms and questionnaires/food logs/other assessment material on a timely basis so productive consultation can occur.
All forms must be returned at least three days prior to your consultation.
If the forms are not returned within the specified timeframes (3 days for Initial consultation and 2 days for follow up consultations), the consultation will be cancelled and no refund shall be given.
You acknowledge you are responsible for the actions you take. You acknowledge that any information provided by us is for informational purposes only and should not be considered to be healthcare advice or medical diagnosis, treatment or prescribing. We encourage you to seek the care of a licensed healthcare professional if you believe such care is required.
You acknowledge that education, information or guidance provided by us should not be construed as a promise of benefits, a claim of cures, or a guarantee of results to be achieved.
No guarantees or warranties are made in relation to our service.
You agree to hold harmless and indemnify Nev’s Health Pty Ltd (ABN 89 662 391 004) and its representatives from any liability whatsoever resulting from your participation in nutrition activities, including but not limited to medical expenses.
You acknowledge that expectations and results or participation in our services vary among individuals and that each individual may not receive the same benefit.

Under no circumstance the consultations are to be recorded (sound or video).

1. When you make your first appointment, we will collect your personal and demographic information via your registration and response to our information packet.
2. During the course of providing services, we may collect further personal information.
3. We may also collect your personal information when you visit our website, send us an email or SMS, telephone us, or make an online appointment.
When, why and with whom do we share your personal information?
We will not share you personal information unless required.
Instances where this may be required may include:
• when it is required or authorised by law (eg court subpoenas)
• when it is necessary to lessen or prevent a serious threat to a patient’s life, health or safety or public health or safety, or it is impractical to obtain the client’s consent

Only people who need to access your information will be able to do so. Other than in the course of providing services or as otherwise described in this policy, we will not share personal information with any third party without your consent, unless you have agreed otherwise.
We will not share your personal information with anyone outside Australia (unless under exceptional circumstances that are permitted by law) without your consent, unless you have authorised us to share your information for research or education purposes, which may be shared with international studies. We will not use your personal information for marketing any of our goods or services directly to you without your express consent. If you do consent, you may opt out of direct marketing at any time by notifying us in writing to info@nevshealth.com.
